Self-Assessment Checklist

Sometimes a challenge you face is clear, simple to understand and easy to discuss with your therapist. However, sometimes there are underlying issues that represent the roots or causes of a problem or challenge. Although we are prepared to address the clear challenges that are well known to you, we are also prepared to assist you to recognize root-cause issues that may need to be clarified before proceeding to a target of therapy.

To assist you in framing the challenge you want to address with Hypnotherapy, we have prepared a simple and straightforward Self-Assessment Checklist that you can download and complete prior to your consultation. Since your responses are not recorded online, or on our website, your privacy is assured.

Tips & Tools

  • Only light-to-medium hypnosis is required to enjoy many of the benefits that Hypnotherapy has to offer. Most people will not need to experience a deep trance-like state of hypnosis.
  • Hypnosis is an altered, yet normal, state of mind. You can be taught and trained to more easily enter this state of mind.
  • The ability to go more deeply into this normal state of mind is a matter of training and practice.
